Welcome to Crescent! Your privacy is important to us, and we are committed to protecting your personal information. This Privacy Policy (“Policy”) governs your access to, and use of the Crescent website portal and application (collectively, the “Services”) provided by Vessel Partners Inc., its healthcare affiliates and professional providers (referred to herein as “Company”, “Us”, “Our” or “We”). This Policy describes the information collected, how we use and share that information, who will have access to the information we collect, and the security measures we offer to protect your information. This Policy applies to information we collect when you access or use our website portal (“Portal”) and application (“App”) (the Portal and App may hereinafter collectively be referred to as the “Platform”), when you use our Services or when you otherwise interact with us.
When you create, register or log into an account through the Platform, you are automatically accepting and agreeing to the most-recent version of this Privacy Policy, as well as to the most recent Portal and the App Terms and Conditions.
Similarly, by visiting, accessing or using the Platform, you are automatically accepting and agreeing to the most recent version of this Privacy Policy, as well as the Portal’s and the App’s Terms and Conditions, and your continuing visit, access or use of the Portal or the App reaffirms your acceptance and agreement in each instance.
Our privacy practices are subject to the applicable laws of the places in which we operate. You will see additional region-specific terms that only apply to customers located in those geographic regions, or as required by applicable laws.

CONFIDENTIALITY OF HEALTH INFORMATION
We receive personal information that may constitute Personal Health Information (defined as information Company receives and/or creates about you, personally, relating to your past, present, or future health, treatment, or payment for healthcare services, Company will treat this information as possible “protected health information” under HIPAA (Health Insurance Portability and Accountability Act of 1996) and the HITECH Act (Health Information Technology for Economic and Clinical Health Act. Your health information may also be protected by different state privacy laws and regulations in the States in which we operate. For users of Services in Canada, Company will treat this information as possible “protected health information” under PIPEDA (Personal Information Protection and Electronic Documents Act) and PHIPA (Privacy of Personal Health Information Act). We consider personally identifiable information that is linked or reasonably capable of being linked to a consumer and that a regulated entity uses to identify the past, present or future health status of the consumer to be consumer health information, and it will be safeguarded as such.
When transmitting and using your health information, Company complies with all applicable laws. In situations wherein you have authorized Us to transmit information to you by other means, we comply with HIPAA, HITECH, PIPEDA, PHIPA, and other relevant state laws and regulations by entering into Business Associate Agreements with the treatment providers for which we provide services to ensure that your protected health information is appropriately safeguarded.

The Right to Opt Out of Cookies and Sale/Sharing Using Online Tracking Technologies

You can opt out of being tracked by these third parties on our Platform by canceling your subscription to our Platform. We do target advertising, but we do not sell your personal information. Residents of Canada can control the use of cookies and other online tracking technologies by reconfiguring their browser, but please note that some or all of the features of the Platform may be limited, and that Company does not guarantee, nor is Company responsible for any continued use of cookies or other online tracking in such instance. You can opt out of being tracked by these third parties on our Platform by canceling your subscription to our Platform, which is the only way to guarantee tracking technologies will no longer be in use. The Right to Limit the Use of Sensitive Personal Information As mentioned above, we only collect sensitive personal information, as defined by applicable privacy laws, for the purposes allowed by law or with your consent. We do not collect or process sensitive personal information for the purpose of inferring characteristics about you.
Some of the purposes for the collection and processing of sensitive personal information allowed by law include: to perform the services or provide the goods reasonably expected; to prevent, detect, and investigate security incidents; to resist malicious, deceptive fraudulent, or illegal actions directed at the Company and to prosecute those responsible for those actions; to ensure the physical safety of natural persons; for short-term, transient use; to perform services on behalf of the Company; and to verify or maintain the quality or safety of a product or service provided by the Company.
Therefore, we do not offer the right to limit the use of sensitive personal information.
The Right to Access, Correct, and Delete Personal Information Depending on your state of residence in the U.S., you may have the right to request access to and receive details about the personal information we maintain about you and how we have processed it, correct inaccuracies, get a copy of, or delete your personal information. You may also have the right to withdraw your consent to our processing of your personal information. If you are a resident of Canada, you have the right to request access to your personal information, to have your personal information rectified if it is inaccurate, incomplete or out of date, and to withdraw your consent to our collection, use, and disclosure of your personal information (including or use or sharing of your personal information for marketing or other secondary purposes by canceling your subscription and use of our Platform. You may also correct basic information through your Client Profile in the Platform you created or, if applicable, by calling your referring medical affiliate.
Typically, we retain your personal information for the period necessary to fulfill the purposes outlined in this privacy policy, unless a longer retention period is required or permitted by law. Please note that in many situations we must retain all, or a portion, of your personal information to comply with our legal obligations; resolve disputes; enforce our agreements; protect against fraudulent, deceptive, or illegal activity; or for another one of our business purposes.
You may submit a request for a copy of the information collected about you by following the steps outlined in the “Your Rights” section above. Depending on your state of residence, you may request for a copy of the information up to twice per year per customer.
Responding to Requests Once you have submitted your request, we will respond within the time frame permitted by the applicable law. Please note that we may charge a reasonable fee for multiple requests in the same 12-month period, as permitted by law.
Appealing Requests Depending on your state of residence in the U.S., you may be able to appeal our decision to your request regarding your personal information. To do so, please contact us in any of the ways listed in the "Communicating With Us" section of this privacy policy. We respond to all appeal requests as soon as we reasonably can, and no later than legally required. If you are a resident of Canada, you may have the right to complain to a privacy commissioner about our decision with respect to your request.
Identity Verification For us to process some requests, we will need to verify your identity to confirm that the request came from you. We may contact you by phone or e-mail to verify your request. Depending on your request, we will ask for information such as your name, an e-mail address or phone number you have used with us.
Authorized Agent Depending on your state of residence in the U.S., you may designate an authorized agent to submit a request on your behalf to access or delete your personal information. To do so, you must: (1) provide that authorized agent written and signed permission to submit such request; and (2) verify your own identity directly with us. Please note, we may deny a request from an authorized agent that does not submit proof that they have been authorized by you to act on your behalf.
If you have been designated as an authorized agent to submit a request on behalf of another customer, you must make the appropriate selection when you submit a Request Form.
Managing Communication Preferences You can stop receiving promotional e-mails, text messages or in-app push notifications by canceling your subscription to the Platform, or notifying us of your desire to cancel your subscription through email at: contact@vesselpartners.com or mail at: Vessel Partners, Inc., 4216 France Ave. S. Right to Non-Discrimination We do not discriminate against customers who exercise any of their rights described in our privacy policy.
Limited Processing of Canadian Data in the United States Our Company is located in the United States but our Platform is used by affiliates and service providers that may be located in Canada, for the benefit and use by Canadian residents. If you are a Canadian resident, your personal health information will always be processed and stored in Canada. Please note that in the initial creation and use of the Platform, your phone number will be transferred to the United States and processed in the United States for the limited and narrow purpose of creating your Platform. Once processed, any number belonging to a Canadian citizen is no longer processed nor stored in the United States.
CHILDREN’S PRIVACY
We provide content on our Platform that is intended for use only by persons eighteen (18) years of age and older. Neither the Site, App or Platform is designed or intended to attract and is not directed to children under 18 years of age. We do not knowingly collect or maintain personally identifiable information from persons under 18 years of age without verifiable parental consent. If we learn that personally identifiable information of persons less than 18 years of age has been collected without verifiable parental consent, then the Company will take the appropriate steps to delete this information. Additionally, our Sites are not intended for or directed to the residents of Colorado, Connecticut, Delaware, Maryland, or New Jersey who are under the age of 18 years. For these states, if we become aware that anyone under the age of 18 has provided us with personal information, we will delete the information from our records. The information automatically-collected by our use of electronic technologies and cookies helps us better understand our users by monitoring how many people are using our website and to improve or tailor our Services by tracking your navigation habits, storing your authentication status so you do not have to re-enter your credentials each time you use our Services, customizing your experience with our Services, fraud prevention and analytics. In addition, use of our Site or App on your mobile device may track the data and activities occurring on and through it, as well as applications installed on it, and include the collection of some personal information such as your media access control, location and traffic data during the time of use. Depending on how you engage with Us, electronic technologies may also be used as navigation aids and as session timers. Our use of electronic technologies and cookies by as outlined above DOES NOT retain Personal Health Information about you.

If you reject our Cookies, certain functions and conveniences of our Site, App and/or Platform may not work properly but you do not have to accept our Cookies in order to use our site. We do not link Non-Personal Information from Cookies to Personally Identifiable Information without your permission and do not use Cookies to collect or store Personal Health Information about you.
OPT-IN POLICY

By creating your client profile and using the Platform you are indicating your consent to “opt-in”.
You may choose to “opt-out” or subsequently change your consent from “opt-in” to “opt-out” and thereby choose not to receive additional marketing information or choose not to allow our sharing of your personally identifiable information by canceling your subscription to the Platform and no longer using our services. THIRD-PARTY PLATFORMS AND SOCIAL MEDIA
Our Sites may contain links to other sites whose information practices may be different than ours.
